Foothills Compassionate Care and the Lay Pastoral Care Team at UU Community of the Mountains are co-hosting a Five Wishes Workshop to help you make important health care decisions for yourself or your loved ones.
The workshop will help you to: talk openly about your preferences while you are able to make decisions; address legal, medical, physical, emotional, and spiritual care arrangements; ease the burden on loved ones by ensuring your choices reflect what matters most; go beyond a basic advance-care directive by including loved ones in your planning.
Every participant will receive a free Five Wishes Toolkit that will lead you through planning and crafting an advance directive with heart and soul, plus guide you in having meaningful conversations, and answer common legal questions.
