Please note that THIS FORM is only to reserve use of a space/zoom room/A/V equipment at UUCM. This information goes onto our 'Building Use' Calendar and is listed from the time entering the room till exiting (not event time(s). To have your event listed in the published church website calendar please complete the announcement form ( This form is used to request space for CHURCH-RELATED ACTIVITIES ONLY (classes, meetings, events, worship, zoom room, lawn use for events/activities, etc.) For any event not church-related, please contact our Office Manager at

Enter the title of the event/meeting/class
Request Type(Required)
If you are making modifications for an event/meeting/class that HAS ALREADY BEEN scheduled, select the update option and add any additional info in the comments section at the end (i.e. cancel room scheduled for 11/11)
This should be the committee/team/group overseeing the organization of the event/meeting/class

Beginning time for ROOM(Required)
This is not the START time for the event. This is the time you need to enter the room for set up.
Ending time for ROOM(Required)
This is not the ENDING time for the event. This is the time that you will be out of the room after clean up.
If reserving a room at UUCM please list your first and second choice. Please enter 'zoom' if you need a zoom room.
